• Begin the narrative with the time when the incident first started.

• Time format - Use the 24 hour system. Two formats can be used - "13:00" or "1300 hrs"

• Date format - MM/DD/YYYY

• Include the callsign you were on duty as/what type of unit you were

• Include where you were when the incident started

• Include details of what happened

• Include descriptions of people and vehicles

• Include descriptions of weapons

• Include the reasons as to why you had Reasonable Suspicion or Probable Cause

• Names format - First name LAST NAME e.g. John SMITH

• Other Police Officers

  • Use short hand for their rank if feasible e.g. PO. I; PO. II; PO. III; Sgt (I or II); etc. Sometimes not as feasible such as Deputy

  • Only need to include last name

  • Add badge number to the end of their name

Arrest Report.

At 1300 hrs on 08/26/2022, I was on duty as 6L23, in the Bay City and Aguja area of Vespucci. A 911 call came in regarding a white man, wearing a blue top and black pants, with a firearm in the Vespucci Canals area. I respond to the area.

On Prosperity Street I see a male matching the description walking down between 2 houses. I request for backup and begin a high risk stop of the male. He gets down onto his knees with his hands in the air. At this point, backup has arrived and so we form an arrest team. This arrest team includes myself, PO. III JOHNSON 123, PO. II SMITH 234, and Sgt. I KELLY 345.

The male is taken into custody without further incident. I search the male and find a Gardone Pistol in his waistband. The male identifies himself as Alex YOUNG and admits to waving the gun around in people's faces.

The male is transported to custody and booked in.

Gardone Pistol; unloaded; has no S/N - scratched off.
